At LinkUp2Care, we offer comprehensive nursing services that are mobile, flexible, and culturally safe. As a Nurse Practitioner with decades of experience in remote and community healthcare, Nicole Ramsamy provides expert clinical support that meets clients where they are—whether that’s in their home, community, or via telehealth.
Our nursing care covers a wide range of clinical needs including wound care, injections, catheter management (indwelling and Mitrofanoff), feeding tubes, bowel care, and continence assessments. We also support medication management, education for support workers and families, and assist with navigating referrals or GP appointments.
Our approach is built on trust, respect, and cultural understanding—particularly important when supporting Aboriginal and Torres Strait Islander clients. We take time to build strong relationships with each person we care for, ensuring their treatment is not only clinically effective, but also personally meaningful and comfortable.
Navigating the National Disability Insurance Scheme (NDIS) can be complex. That’s why our Support Coordination service is focused on simplifying the process and empowering participants to take control of their care.
We work directly with NDIS participants and their families to understand their plan, identify suitable services, connect with providers, and coordinate support that aligns with their goals. Whether you’re new to the scheme or already have a plan in place, we guide you through each step, helping to reduce stress and increase outcomes.
Nicole’s deep knowledge of both the health system and cultural context means she can advocate strongly for clients and provide meaningful support that goes beyond administration. Our aim is to build confidence, encourage independence, and ensure every person receives the support they’re entitled to in a way that suits their lifestyle and community needs.
Our midwifery services are designed to support women and families through every stage of pregnancy and early parenthood. With a strong focus on cultural safety, LinkUp2Care provides holistic care that respects family, community, and the unique needs of each mother and baby.
We offer antenatal checkups, education about pregnancy and birth, postnatal support, breastfeeding guidance, and health advice tailored to each family’s situation. Nicole Ramsamy brings years of experience as both a midwife and Nurse Practitioner, ensuring clients receive professional, compassionate care in the comfort of their own environment.
These services are especially important for women in remote or underserved areas who may not have easy access to mainstream maternity care. We aim to create a safe space for women to ask questions, feel supported, and receive high-quality care—close to home and with cultural respect at the heart of every visit.
Managing a long-term health condition can be overwhelming—but with the right support, it doesn’t have to be. At LinkUp2Care, our Health Management services focus on helping clients live well with chronic conditions such as diabetes, cardiovascular disease, kidney problems, and respiratory illnesses.
We offer personalised care plans that include regular health monitoring, lifestyle and medication support, education about the condition, and coordination with GPs or specialists. As a Nurse Practitioner, Nicole Ramsamy can prescribe medications, order tests, and make referrals—streamlining the process and reducing the need for multiple appointments.
Our service is particularly valuable for those who need culturally appropriate, in-home or remote healthcare. We take the time to understand each person’s lifestyle and background, ensuring that care plans are realistic, respectful, and sustainable. Our goal is to empower clients to take control of their health and improve their quality of life over the long term.
